After showcasing the Cleveland United Way-led application of CAPGI for medically tailored meals for socially isolated older adults with certain chronic conditions in the webinar on September 24th (recording here , slides there ), we plan to host two more webinars later this fall. The first, on November 3 (register here ), will highlight lessons learned from numerous Trusted Brokers around the country, with the intent of informing those who are considering similar collaborative journeys. The second, later this year or early in 2022, will shine a spotlight on the DC team, led by the Jane Bancroft Robinson Foundation, which has implemented a comprehensive approach to SDOH intervention design with authentic community engagement through a racial equity lens that is truly unique in our experience.
